Subject: 2021-2022 O.S.F.M. Emergency Preparedness Drill Record Poster & Drill Report Cards



Enclosed are nine (9) Fire/Tornado/Crisis Drill Report Cards (Stock No. 8230-00) and the O.S.F.M.  Emergency Preparedness Drill Record Posters for the 2021-2022 School Year.  Please read the entire K.S.F.M. poster/form before filling in the blanks.  Please fill out the bottom portion of the drill report cards with the enrollment, the name of your school, and your Principal’s signature before sending in Fire/Tornado/Crisis drill report cards.


Fire/Tornado/Crisis Drill Report Cards (9 total drills) should be submitted each month as required by State law.


Tornado Drill Report Cards should be submitted for drills required by Administrative Regulation No. 2275-1 held in September and March.  Reports should also be submitted for any other drills deemed to be appropriate, such as the State-wide drill usually held in March.


Crisis Drill Report Card should be submitted 3 times per school year.


Please pony mail the completed card to Lisa Gay at Central Services.  At the end of the school year, send the O.S.F.M. Emergency Preparedness Drill Record Poster to Lisa Gay by pony mail.
